Solutions for Youth & Family / Tompkins County SPOA

signpost

The Tompkins County Single Point of Access (SPOA)/Solutions for Youth and Families(SFYF) team is a group of dedicated, compassionate human service professionals who are available to support families of children who are experiencing social, emotional and behavioral challenges that impact their success at home, in school or in the community. Families can either refer themselves to the team or a child/youth can be referred by a school or agency with the caretaker's written permission.  With a goal of offering support to a wide variety of families, there are two different kinds of meetings that can be faciliated through the SPOA/SFYF team. These include:

1. A traditional SPOA meeting for which the child/youth typically has a mental health diagnosis and is currently in treatment. Community based outreach services accessible only through a SPOA meeting are Intensive Case Management (ICM) and Home and Community Based Waiver (Waiver). In our county Pathways is the agency that delivers our Waiver services.

2. A Solution Focused Conversaton can be faciliated with either the whole team or with a smaller group of team members. These can be held at the regular thursday morning meeting time or can be held at an alternative time in a place other then the Tompkins County Mental Health Clinic where meetings are typically held. The goal of these meetings is to explore creative ways to support a family from a solution focused perspective. Meetings are facilitated by the team coordinators with a strength based family driven approach.

The referral form for the SPOA/SFYF Team appears below along with a guide for Caregivers who may be requesting a meeting.
